Understanding the Case Fee Model
Our type of care is highly specialized and unique so all potential new patients will make a consultation first. The consultation is 15 minutes and the doctors will briefly discuss your case with you to determine if you are in the right place. If you still have pain, then something was missed in the diagnostic process. You’ve already been through a lot. The last thing you need is another failed treatment plan.
If you are qualified after the consultation, then the comprehensive examination will be scheduled on your next visit. Once a complete diagnosis is discovered and our doctors know they can help, the doctor will go over your condition and treatment plan in detail.
Case Fee (payment plans available)
- Mild Complexity $1,600
- Moderate Complexity $2,100
- Severe Complexity $2,600
**The Price depends on the complexity and severity of your case and will be discussed in detail after your comprehensive exam and diagnosis.
We have an extremely high success rate with our patient cases. Unlike many healthcare providers, our office operates on an improved model of payment called a Case Fee. Rather than charge you for each and every visit we offer a single upfront price for your entire case. This has several big advantages:
1. Upfront and transparent pricing. You know exactly how much your care will cost.
2. Aligned Interest. Patients and doctor have one goal, get you better as fast and safely as possible.
3. Completion of care. Too often many patients drop out of care when they are feeling 50-80% better. Unfortunately, when care isn’t completed, the improvements may not last. Case fee encourages reaching maximum improvements by making an upfront commitment.

Do you offer payment options for treatment plans?
Yes, the details of your payment options will be known after your diagnosis is made and your treatment plan is prescribed.
Do you have a refund policy?
Yes, we do. The details of the refund will be discussed in detail if you are a candidate for care.
What Is the comprehensive exam?
This is where the doctor will get familiar with your case and more details to see if you are a candidate for treatment. You have already been through a lot. The last thing you need is another failed treatment plan.
Typically this appointment lasts 45 minutes and entails an Integrative Diagnosis® exam, neurological testing, accurate diagnosis, report of findings, review of imaging and medical records and provides a treatment plan.
Dr. Nottoli will also discuss any other complicating factors that may be present or could potentially become a problem.
Please come in workout attire, evaluation and treatment are most effective without the impedance of clothes if necessary. For example, if you are coming in with shoulder pain, please wear a tank top. If you have knee or leg pain, wear workout or running shorts.
Please bring any imaging and relevant radiology reports. No treatment is done on this visit.
Do you take my insurance?
Our office does not accept any insurance. Insurance companies don’t care if you get better–they only care about saving themselves money. They also put up arbitrary barriers and ultimately keep doctors from providing excellent care. Insurance doesn’t reimburse for quality or successful outcomes.
Our focus is on YOU and making sure you receive the highest quality diagnosis and treatment possible. This yields incredible results! We do accept cash, check major credit cards, Care Credit, health savings accounts (HSA), and flexible spending accounts (FSA).
How do I know if the doctors will be able to help?
If you have any muscle, joint, or nerve problems then you are a good candidate. This is our expertise. If you have also failed other treatments or haven’t reached your maximum level of improvement, then you are also a good candidate.
Vitality Soft Tissue & Spine specializes in finding and fixing adhesion, which is a difficult problem to find and even more difficult to fix. Integrative Diagnosis® trained providers can help with this problem as it is not extensively trained in any doctorate education.
How long are follow up visits?
Follow up visits are 15 minutes. The goal of any treatment is to get you back to functioning as well as possible without pain and limitation so you will be given specific guidance that you are expected to follow if you want the best resolution to your problem. This is a team approach—not a passive one.
Are there any other treatments that can do the same thing?
The short answer is not exactly. There are many manual treatments and instrument treatments that seem similar to the patient perspective, but the proof is in the results. Sustainable improvements in pain and range of motion are the unique differences that give you results.
How long does a treatment plan last and when will I get pain relief?
On average, a single region of complaint (i.e. shoulder or low back) can reach a full resolution or maximum medical improvement in an average of 12 visits.
We typically see sustainable functional changes that are measured in 1-2 visits. Most patients begin to feel sustainable improvements in pain within 4-6 visits.
